5 stars the first thing I noticed as entering the facility set air quality great I DID NOT notice a stingy smelly urine at all.😃 The past two times that I have been there the place was stench smell of urine everywhere. The new faculty Garden City healthcare wants to bring good health care I’ve experienced a very good session with 98% of the staff I talked to management and they said they will address the issues so we can be 100%. A few people can disrupt the way that the management wants to lead the company. The other thing I would like to mention is that they offer students in training to come and train with the nurses other staff. Therapy they have a good system where there are therapy to get your situation under control so when you leave you’re able to benefit from their healthcare. Food,, staff member from the kitchen came to visit and told me there are other options for people desire other foods that are on the menu. Most of the food was well-prepared. There was only one disappointment a grilled cheese sandwich a certain individual made a decision and decided to serve mean a burnt grilled cheese bad taste took away taste of the rest of the mill. I mentioned it to the kitchen staff person that if there’s any question is to ask their Superior management, they said we will definitely take care of that situation. To end my information on my stay I would have to say please bring your family members to this garden City healthcare it has change to greatly to better and comfort the patients.

I was there for two weeks to recover from surgery and the first night it took 2 hours for them to answer a call to use the restroom. I started to feel ill after eating the food. Drew the physical therapist pushes everyone too much I felt lightheaded and couldn’t stand and he told me he can wait here all day. I keep vomiting for a week till they finally sent me to the hospital and I refused to return there. Beds were too small. Some of the staff was ok but felt that a lot didn’t know what to do, one didn’t know how to use the BP machine. They had high school kids working there also with no experience. Don’t ever go hear

The staff at this facility do what they can to care for the residents the best way possible. Nursing homes are not the most desired places to be and this work is not easy. Housekeeping does great. The therapy is fantastic. There’s always activities going on to keep residents busy and interacting. Nursing is on top of it and the management team works to resolve issues in a timely manner.

Great staff. The Nurses and Therapists were great. If it wasn’t for them I would have requested to be moved to another facility. They had great compassion and empathy considering what they were dealing with on a daily basis from other patients they were dealing with. I myself tried to treat them with the respect they deserved

Garden City Health Center is possibly the best rehab i have been a patient. Key words: very efficient and accomodating. The staff is friendly, and caring. Clean is and understatement. Constantly cleaning. Food is tailored within the meals to accommodate food needs. There are some alternate choices. There are two gyms. Scheduled therapy and they put you thru your paces. I requested this facility because they did my cousins double knee replacement therapy. He was extremely satisfied. 5 star is not enough

I’m giving this a one star because you have to put something down unfortunately. My mom went there after being in the hospital and she was left in isolation from November 30th-till December 8th.She would push her call button and no one would check on her for two hours when all she needed was water! She would call the facility if I didn’t and then someone would come check in on her. They also messed up her medication and she went the whole time without her night time meds and if they did give it to her it was around 3pm and she would ask them to fix it several times and they would lie to her and tell her they were working on it and it never got fixed. They would start handing out lunch trays at 12pm and my mom wouldn’t get her food until an hour later which it was always cold because the hot water heater was broken and they couldn’t keep they plates clean and sanatized so they would serve the food on paper plates, also they are giving patients cold showers two times a week the water heater has been out for about a month! Physcal therapy was awesome and two of the nurses Michelle and Irma were the only two who actually made attempts to take care of my mom and we’re always on the ball to help her so thank you guys for that if it wasn’t for those two ladies my mom would of been forgotten about!

After my mother broke her ankle and ended up with a bone infection in her foot, she was in need of SNF support and care during her elongated recovery. Because she was visiting from out-of-state, there were challenges getting through the health insurance bureaucracy to obtain the necessary authorizations. Jodie, at Garden City was very kind, helpful and supportive in getting through the ‘red tape’; responsive to us as family members; and acted as a liaison to other staff when my mom was struggling and needed extra attention. The facilities appeared clean and cared for. All the staff was very patient and kind, providing my mom wonderful support and care. It means a lot to us as a family to know our vulnerable loved one was in caring, responsive and competent hands during her recovery. Thank you!

I cannot say enough praises for this facility. My mother-in-law was there for 3 weeks rehabing from knee replacement surgery. I always had tons of questions every day about her health maintenance, etc. and the staff could have easily got frustrated with me. Never once did I experience a cranky employee, and if one didn’t know the answer, they referred me to who did. They basically have an open door policy for all their departments which is great. My mother-in-law got discharged yesterday and I owe everything to Garden City for her great rehab experience! God bless you all for what you do to help the elderly.

I agree with many of the negative comments regarding cleanliness and subpar care. My mother broke her femur on Saturday October 4 and had a rod installed late afternoon on Monday October 6. On October 7, she was transferred to Garden City at approximately 5pm to recieve physical therapy to be able to walk again (She had not stood on her leg since breaking it). She was placed in the middle bed of a three bed room There were issues from moment one including giving her food she was allergic to. I called immediately and left a message to have the administrator call me first thing in the morning. During the night my mother waited for over an hour to have her diaper changed. At 8AM (10/8) I called and asked again for an administrator to call me back. At 10:30 I arrived at the facility and was assured someone would come talk to me. I also expressed my concern that equipment from the other two beds were encroaching on her area greatly decreasing the walking area on the sides of my mother’s bed (14 inches on one side and 22 inches on the other) and I even tripped twice (I am 100% disabled). Please see attached photos. At approximately 11:30 am , I stepped out for a moment. While I was gone, staff was sent to take an xray and the nurse insisted she get up and walk (without any aid) approximately 10 feet because the portable xray machine did not have enough room to be next to her bed. After this happened she was in extreme pain and her pain pill was not give for another hour. I am extremely concerned that the administration would refused to talk to me and that a nurse would make a patient walk before being evaluated by Physical Therapy.i had her transferred that afternoon to another facility. I would not let these people care for my pets.
The experience at Garden City Healthcare was exceptional. From the administrative staff to the medical providers, the experience was exceptional. Everyone was professional and attentive, and the patients are genuinely cared for. They clearly go above and beyond for their patients. The facility is highly recommended for those seeking top-quality care.
I cannot say enough positive things about Garden City Healthcare Center in Modesto, CA! From the moment we arrived, the staff made us feel welcomed and cared for. The facility is exceptionally clean, well-maintained, and has a warm, inviting atmosphere that immediately put my family at ease. The nursing team is outstanding—compassionate, attentive, and truly dedicated to providing personalized care. My loved one received top-notch rehabilitation services, and the physical and occupational therapists were phenomenal, helping them regain strength and independence with patience and expertise. The 24/7 nursing care ensured all needs were met promptly, and the staff always took the time to answer our questions and keep us updated. The facility itself is impressive, with a fully equipped therapy gym, engaging activities, and nutritious meals tailored to dietary needs. The spacious courtyards and common areas added a sense of community and comfort. It’s clear that Garden City prioritizes both the physical and emotional well-being of their residents. I wholeheartedly recommend Garden City Healthcare Center to anyone seeking skilled nursing or rehabilitation services. They truly go above and beyond to ensure a supportive and caring environment. Thank you to the entire team for making such a positive difference!
The bed fell apart while family stayed in rehab. I had to help a nurse on the long term side while a loved one is still there. They do try, facility not as clean as it should be.

I am very surprised at the negative postings on this beautiful facility . It is not what I have recently encountered. From the admission process, the daily nursing care and social services involvement , I would come to this place if need this level of care. It’s a privilege to have my friend there.

My Dad was just recently discharged from Garden City. I would first like to say the care he received was amazing, the staff went above and beyond to meet the needs he needed. The cnas and Lvns treated him with so much respect The physical therapy was just as amazing. They got my dad back up and walking.

If no star’s was an option that’s what this place would get. My mother was here. She was treated horribly. They were abusive towards her. She’s in the hospital because she got pneumonia. She was able to walk before and now walking doesn’t look like it will be an option for her anymore.

A family member of mine was participating in short term rehab at Garden City recently and cannot express how thankful we are for all they did. The staff was attentive, patient, responsive and I am so thankful for each and every one of them. Thank you everyone that works there for all you do. A special Thank you to Jackie (nurse and liaison). She was on top of every question and concern and made the transitions seamless.

This facility should be shut down. Poor cleanliness, falling apart. Depressing seeing the patients roam around, female patients in wheelchairs roaming in male rooms. No supervision. Very bad experience for my Dad and my Family.

We recently had our father at Garden City, and while the staff includes some truly exceptional professionals, unfortunately the condition of the facility itself left us no choice but to transfer him elsewhere. Let me start by highlighting what did go well — the physical therapist, occupational therapist, and speech therapist were absolutely phenomenal. They were compassionate, knowledgeable, and deeply committed to my father’s recovery. The receptionist was also warm, professional, and helpful, making the front-end experience as smooth as possible. These individuals are the heart of this facility and deserve real recognition. However, the overall state of the building is extremely disappointing. It is outdated, worn down, and gives off an impression of being unclean, even if it technically is. The environment feels neglected, with décor and infrastructure that clearly haven’t been updated in decades. This contributed to a general sense of discomfort and concern for our loved one’s well-being. It’s hard to feel at ease placing someone you care about in a place that looks like it’s falling apart. The nursing staff was okay, but seemed to be spread very thin, likely due to staffing shortages, and you could tell they were doing their best under difficult conditions. They weren’t rude or unprofessional, but they were clearly operating at capacity with many high-needs patients. Ultimately, while the people working here are kind, capable, and dedicated, the facility itself is not fit for the level of care being provided. It’s unfortunate, because the right infrastructure could make this place outstanding. But as it stands, we felt compelled to move our father to another nearby facility where his dignity and comfort could be better preserved. We hope the ownership seriously considers investing in major renovations, because the staff here deserve a workplace that reflects their excellence, and patients deserve an environment that promotes healing and respect.

I’m highly recommend finding a different place for your loved ones to stay with and recuperate at. I visited recently and found most of the staff to be extremely unsupportive and rude. I have asked for a call back about my father’s conditions multiple times and have gotten one call from a night nurse that said someone would call me back in the morning and it never happened. My father has experienced theft and general neglect at this facility with no response. I recommend moving your loved one or never coming here in the first place.
